Your first appointment consists of a full review of your pet’s history, diet, and physical examination. This allows the doctor to gain a whole picture of your pet’s health. As part of your visit, you will receive 2 complimentary books that focus on natural living and healing from home.
To help us provide the highest level of care, we ask you send us the following information or notify of a change in schedule at least 48 hours in advance of your appointment.

Cancellation Policy
We feel honored to have you and your companion animals as a part of our family. When a client cancels without giving enough notice, they prevent another patient from being seen. Please call us at 508-435-4077 at least 2 days (48-hours) prior to your scheduled appointment to notify us of any changes or cancellations. If prior notification is not given, you will be charged $100 for the missed appointment. MASH is committed to providing all of our patients with exceptional care.
Payment in full is required at the time of your visit. We accept cash, check, credit card, and Care Credit.

- Medical Records Release Form: We required all previous records including Medical History, Vaccine History and Medication History. Your records will not be released to us without this form. If you prefer, you can request your current provider to have all records emailed to [email protected] or faxed to 508-435-5533.
